Donation Request

The Monticello Lions Club is proud to support a variety of community programs and efforts that improve lives. Requests for donations can be made by completing and submitting the following Request for Donation Form.

Our club has a limited budget for charitable giving, therefore, not all requests can be awarded. Our membership reviews requests on the first Monday of every month, please submit before the previous month's end to be considered.
